How to sign up for NextMD

There are three ways you can sign up:

Option 1: Sign up online. Visit

Option 2: At your next visit to USMD, tell us you want to sign up. We’ll give you a temporary token. Use that token to sign up at

Option 3: Call us at 1-800-555-1429 and press option 3. We’ll help you sign up.

How to sign up for FollowMyHealth

There are two ways you can sign up:

Option 1: At your next visit to USMD, tell us you want to sign up. We’ll help you get started.

Option 2: Send an email to ­ ­We’ll send you an email invitation to set up your username and password for your online health record. ­ ­ 

When you register, you’ll be asked to enter an invitation code. Use the last 4 digits of your Social Security number. If you don’t wish to use your Social Security number, use “1111.” Please note: the code is only active for seven days. ­ ­

  • What are the browser requirements?
    You’ll need:

    • One of these browsers: Internet Explorer 7.0 or higher, Mozilla Firefox 2 and 3, Google Chrome 4, or Safari 1 and 
    • One of these operating systems: Windows or Mac
    • Microsoft Silverlight 4.0 (you’ll get a prompt if it’s not installed on your system)
    • A Windows Live ID (or Hotmail)
  • When you sign up with us to set up a FollowMyHealth account, we’ll give you a link. Once you’ve signed up, you can click on the FollowMyHealth link above. Or visit
  • Go to the “My Doctors” tab to see if your provider accepts appointment requests. If the provider does, you’ll see a “Request Appointment” link under the provider’s photo.

    Click on the “Request Appointment” link. Choose the date you want, and click to select it.* Then type the reason for your visit. You can add any special instructions, as well.

    *To choose multiple dates, hold the Ctrl key and click the dates you want.

  • Yes, absolutely. Go to the “Messages” tab, and click the “Compose a new message” icon. If your provider accepts messaging, you’ll be able to choose that provider from the drop down. Type your message and click the “Send” button.
  • Once you make the payment, it could take up to 24 hours for the payment to show in FollowMyHealth.
